Student Health and Wellness maintains a Staff Directory as part of our website. By clicking on a person in the directory, you will be brought to that individual's Person Page.

Whether you are an employee who needs to be added to the directory, or your would like to update information already in the directory, please use the form below to provide the information needed to create your Person Page.

  • If you are using this form to update existing information, please put 'NC' or 'No Change' in the fields that do not need to be changed.
  • If a field doesn't apply to you, please enter 'NA.'
  • If you are unsure about what to enter for any of the fields, look at current directory entries for examples.

  • Suffix = degree, licensure, certification (i.e. MS, PhD, APRN, LPC, RD, etc.)
  • This can be the official university/union title and/or your internal/functional title used on SHaW org charts.

  • (primarily professional or clinical; can include personal if desired and appropriate)
  • Photo should be a minimum of 3" x 3". A "head shot" is preferred. The following image types can be accepted: .png, .gif, .jpg, or .jpeg.
